What is the difference between medical billing and RCM?
Medical billing is preparing and sending claims. RCM covers the whole payment journey, from registration to final payment, including denials, A/R and reporting.
Why are claims denied so often?

What is the difference between a rejected and a denied claim?
A rejected claim is stopped before the payer processes it, usually because of an error, and can be corrected and resent. A denied claim was processed and refused, and usually needs a correction or an appeal.
